AN UPDATE ON MECHANISTIC MODES, DIAGNOSTIC APPROACH & DECIDE FOR SURGERY/VIV IN EARLY BIOPROSTHETIC VALVE FAILURE; A REVIEW

ABSTRACT
The use of bioprosthetic heart valves has expanded considerably over the last 20 years, now are contributors of the maximum of valve replacements conducted worldwide. Whereas late structural valve degeneration continues to be the dominant mode of failure further than 10 years, early bioprosthetic valve failure (EBVF), defined as valve impairment making rearbitration imperative, resulting in robust haemodynamic diminishing or demise amongst 5 years of implantation, delineates a unique as well as escalatingly acknowledged clinical botheration. EBVF is multifactorial in origin in addition to portrays the interaction of three fundamental pathological events: i) accelerated structural valve degeneration guided by host immune reactions along with ii) dystrophic calcification; iii) valve thrombosis, both subclinical as well as clinical, inclusive of hypoattenuated leaflet thickening following transcatheter implantation; as well as prosthetic valve endocarditis, which carries unproportionate early mortality. This review synthesises present insight i) of the mechanistic modes, ii) risk factors, iii) diagnostic strategy, along with iv) therapeutic implications of EBVF, with emphasis on the surgical viewpoint. A temporal-mechanistic strategy—associating time from implantation to plausible pathology—possesses the capability of refining diagnostic exactitude as well as guide correct selection of i) anticoagulation, ii) antimicrobial therapy, iii) redosurgery or iv) valve-in-valve arbitration.
