THREADS OF INNOVATION: A SYSTEMATIC NARRATIVE REVIEW OF THE EVOLUTION IN LIVER SUTURING TECHNIQUES

ABSTRACT
Discussion: The history of liver suturing has changed over the past century. Liver suturing was developed as a way to manage haemorrhage during liver surgeries, going through cycles of innovation, and decline in popularity. Hailed as effective, quick, and simple to perform, liver sutures are a part of surgical practice that has both historical and modern applications that require specific focus. This review objectively defines these techniques and their relevant applications in modern practice. Methods: Systematic review of literature was made following the PRISMA guidelines. Studies describing the modern application of modern liver suture techniques in its outcome, and the cases highlighting the same with details were pursued through electronic databases. History with regard to liver suturing has been reviewed and recent developments in this area are presented. Results: Eighty-eight articles were included in the review from an initial pool of 256 articles. Compressive suture was early technique which later developed to utilization of spade needles and special materials. Innovations include such key items as the transfixing suture, Kousnetsoff-Pe?ski, and modern variants of Chang’s needle. Liver suturing, almost switched to use of more sophisticated surgical instrumentation, is of relevance in resource limited settings as well as in certain specialized situations. Conclusion: Though antiquated in some respects, liver suturing offers value to the field of surgical innovation and is a viable choice when a liver transfer is indicated. Refinement and adaptation could improve its use in modern hepatobiliary surgery.
